School of choice for kindergarten only is open from May 1st to May 31st.
Henry Ford Jackson Sports Medicine is excited to conduct sports physicals for students planning to participate in athletics during the 2023–2024 school year. All locations are open to students from any school district needing a sports physical. No appointment is necessary.
Students must bring: • Sports physical and waiver forms completed and signed by a parent. The physicals cannot be conducted without these forms. Sports physical forms and waivers can be downloaded on our website. • Cash payment is $20. Payment is accepted at the time of the sports physical. Please note, athletes who are not accompanied by parents can only be seen by the physician if all forms are completely signed. The schedule of available locations and times for sports physicals is to the right. Choose the location that best suits you.
